You need the following forms / documents and then take the fully completed forms to UIF if you want to claim UIF for retrenchment

a. Clear Copy of ID of employee
(ID cards must be front and back on one page)
b. UI2.1 completed and signed by employee
c. Termination notification from employer
d. UI8 completed and signed by employee
e. UI19 completed and signed by employer
